  Attached please find the vacation house check form.  Fill out the form completely with as much information as you can provide.  You are required to bring this form in person to the Alpharetta Police Department lobby so that we can verify that you (the resident) are the person requesting this vacation house check.  Once the receptionist has the form it will be forwarded to the communications division and given a computer aided dispatch (CAD) number.  This will ensure that your address is not given out over the police radio.  Once the supervisors receive the form, an officer from each squad (two times a day) will check the perimeter of your house and make sure that it is secure.  This will reoccur daily until the discontinuation date that you have provided. 

Important note- If you return early from your vacation, you must notify the Alpharetta Police Department communications division (678) 297-6300 so that the house check can be cancelled.
